Overview

Platelet-rich plasma (PRP) is an autologous biological treatment derived from a patient's own peripheral blood through centrifugation to concentrate platelets. In the context of the AURoRA clinical trial (NCT02678936) conducted by the University Hospital, Basel, PRP is evaluated as an adjunct to mucosal advancement flap surgery for the treatment of complicated anal fistulas. The therapeutic effect is driven by the alpha-granules within the concentrated platelets, which release a variety of growth factors, including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF). These factors act on their respective receptors to promote angiogenesis, fibroblast proliferation, and collagen synthesis, thereby accelerating the healing of the fistula tract and potentially reducing the high recurrence rates associated with standard surgical interventions.
