Platelet-rich plasma (PRP) is an autologous biologic therapy consisting of a concentrated suspension of platelets derived from a patient's own blood. In a clinical study led by the University of North Carolina, Chapel Hill, PRP is being investigated as a treatment for partial thickness rotator cuff tears, specifically using the Arthrex ACP (Autologous Conditioned Plasma) preparation system. The therapeutic effect is mediated by the release of high concentrations of growth factors and cytokines from platelet alpha-granules upon activation, including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF). These factors promote tissue repair by stimulating fibroblast proliferation, collagen synthesis, and angiogenesis at the site of the tendon tear. The UNC trial is a randomized, double-blind study comparing the functional outcomes and healing rates of ultrasound-guided PRP injections versus traditional corticosteroid injections.
