Platelet-rich plasma (PRP) combined with human insulin is a therapeutic intervention studied for the treatment of atrophic acne scars, specifically within a clinical trial framework at the Jinnah Postgraduate Medical Centre (JPMC). This approach utilizes microneedling (via a dermaroller or dermapen) to create controlled micro-injuries in the skin, which are then treated with either autologous PRP or topical human insulin. PRP is a biologic product derived from the patient's own blood, concentrated to provide a high density of platelets that release growth factors such as platelet-derived growth factor (PDGF), transforming growth factor beta (TGF-beta), and vascular endothelial growth factor (VEGF) to promote tissue regeneration and collagen synthesis. Human insulin, when applied topically in this context, acts as a peptide hormone that stimulates collagen formation and angiogenesis, potentially accelerating the remodeling of depressed scars. The JPMC study compared these two adjuncts to microneedling, concluding that the insulin combination provided superior clinical outcomes for atrophic scarring compared to the PRP combination.
