Overview

Platelet-rich plasma (PRP) combined with percutaneous meniscal trephination is a biologic intervention developed for the treatment of meniscal tears, specifically chronic degenerative lesions. PRP is an autologous blood product containing a high concentration of platelets that, upon activation, release a variety of growth factors including platelet-derived growth factor (PDGF), transforming growth factor-beta (TGF-β), and vascular endothelial growth factor (VEGF). These factors promote cellular proliferation, chemotaxis, and angiogenesis, facilitating tissue repair. Trephination is a procedure involving the creation of multiple small channels in the meniscus using a needle, which stimulates a localized healing response and improves vascular access to the typically avascular zones of the fibrocartilage. This combined approach, investigated by the Centre of Postgraduate Medical Education in Poland, aims to enhance the regenerative capacity of the meniscus by providing both mechanical stimulation and a concentrated supply of bioactive proteins.
