**Plinabulin + pembrolizumab + docetaxel** is an investigational combination regimen being evaluated for the treatment of patients with metastatic non-small cell lung cancer (NSCLC) who have progressed on prior immune checkpoint inhibitor (ICI) therapy. The regimen combines three distinct therapeutic agents: plinabulin, a first-in-class small molecule tubulin-binding agent and GEF-H1 activator; pembrolizumab, a programmed cell death protein 1 (PD-1) blocking monoclonal antibody; and docetaxel, a standard-of-care taxane chemotherapy. Plinabulin acts as a selective immunomodulating microtubule-binding agent that promotes dendritic cell maturation and T-cell activation via the GEF-H1 pathway, while also mitigating docetaxel-induced neutropenia. Pembrolizumab prevents PD-1 from interacting with its ligands (PD-L1 and PD-L2) to restore anti-tumor T-cell activity, and docetaxel directly inhibits cell division by stabilizing microtubules. Together, this triple combination aims to synergistically enhance the cancer-immunity cycle, overcome acquired resistance to immunotherapy, and improve clinical outcomes in the second- or third-line setting.
