The combination of polidocanol and sodium tetradecyl sulphate (STS) consists of two distinct detergent sclerosing agents used primarily in the treatment of varicose veins, telangiectasias, and venous malformations. Polidocanol is a non-ionic detergent, while sodium tetradecyl sulphate is an anionic detergent. Both agents function by inducing local endothelial damage through the disruption of cell membranes (detergent action). This chemical irritation triggers platelet aggregation, thrombus formation, and subsequent inflammation, eventually leading to the replacement of the vessel with fibrous tissue (sclerosis) and permanent closure. In clinical practice, this combination is often employed in 'dual sclerotherapy' or as a compounded mixture to enhance efficacy in larger or more resistant vessels compared to monotherapy with either agent alone.
