poly(lipoic acid trisulfide) (pLATS) is an oral polymeric nanomedicine developed for the treatment of hypertensive chronic kidney disease (CKD). It is designed to overcome multiple biological barriers through trisulfide exchange with thiols at mucosal, epithelial, and renal cellular interfaces. Upon entering cells, pLATS undergoes thiol-triggered depolymerization, providing a sustained release of hydrogen sulfide (H2S) and lipoic acid (LA) or lipoic acid trisulfide (LATS) for over 60 hours. This dual-action mechanism involves H2S-mediated regulation of the renin-angiotensin system and endothelial function, alongside LA-induced upregulation of endogenous H2S-producing enzymes. Preclinical studies indicate that pLATS effectively reduces blood pressure and improves renal biomarkers, such as the urine albumin-to-creatinine ratio and plasma urea levels, showing superior efficacy compared to the standard antihypertensive losartan.
