Poly-ICLC + pembrolizumab is an investigational combination therapy consisting of poly-ICLC, a synthetic double-stranded RNA that acts as an immune adjuvant and Toll-like receptor 3 (TLR3) agonist, and pembrolizumab, a monoclonal antibody checkpoint inhibitor targeting programmed cell death protein 1 (PD-1). Poly-ICLC stimulates innate immunity by activating TLR3 and MDA5 pathways, leading to the production of type I interferons and pro-inflammatory cytokines, enhancing dendritic cell maturation and T-cell activation. Pembrolizumab blocks PD-1 on T cells to prevent tumor-induced immune suppression. The combination aims to enhance antitumor immune responses by both stimulating innate immunity (poly-ICLC) and relieving adaptive immune inhibition (pembrolizumab). This regimen has been studied in clinical trials for various cancers including mismatch repair proficient metastatic colon cancer, glioblastoma multiforme (GBM), non-Hodgkin lymphoma, among others[2][7][4].
