poly-ICLC + temozolomide is a combination therapy used primarily in the treatment of glioblastoma. Poly-ICLC (polyinosinic-polycytidylic acid stabilized with poly-L-lysine and carboxymethylcellulose) is a synthetic double-stranded RNA that acts as an immune modulator by activating innate immune responses through Toll-like receptor 3 (TLR3) and MDA5 pathways, leading to the production of type I interferons and other cytokines. Temozolomide is an oral alkylating agent that induces DNA damage in tumor cells by methylating guanine residues, resulting in cytotoxicity especially in rapidly dividing cells. The combination has been studied for its potential synergistic effects—temozolomide provides direct antitumor activity while poly-ICLC enhances antitumor immunity. This regimen has been evaluated alongside radiotherapy or peptide vaccines for newly diagnosed glioblastoma patients[1][2][3][4][6].
