This investigational topical combination therapy, developed by the University of Virginia, consists of polymyxin B, nystatin, and nitrofurantoin formulated in a Pluronic F68 (poloxamer 188) gel base. It is designed for umbilical cord care in neonates admitted to the Newborn Intensive Care Unit (NICU) to prevent bacterial and fungal colonization of the umbilical stump, thereby reducing the risk of invasive bloodstream infections. Polymyxin B provides activity against Gram-negative bacteria by disrupting the outer membrane, nystatin acts as an antifungal by binding to ergosterol, and nitrofurantoin offers broad-spectrum antibacterial activity through the formation of reactive metabolites that damage bacterial components.
