Polymyxin b sulfate + hydrocortisone acetate + neomycin is a fixed-dose small-molecule anti-infective and anti-inflammatory combination used in topical, otic, and ophthalmic products for corticosteroid-responsive inflammatory conditions with concurrent or threatened bacterial infection. Neomycin is an aminoglycoside antibacterial that binds the bacterial 30S ribosomal subunit and inhibits protein synthesis, polymyxin b sulfate is a polypeptide antibacterial that disrupts the outer membrane of susceptible Gram-negative bacteria through interaction with lipopolysaccharides and membrane phospholipids, and hydrocortisone acetate is a corticosteroid that activates the glucocorticoid receptor to suppress inflammatory gene expression and reduce edema, erythema, and pruritus. The combination is best known through the Cortisporin franchise and related generic products, historically associated with Monarch and Pfizer, and has been used for indications such as corticosteroid-responsive dermatoses with secondary infection risk, otitis externa, and inflammatory ocular conditions where antibacterial coverage is desired.
