Overview

**pomalidomide + carbamazepine** is a combination of two distinct small molecule drugs: **pomalidomide**, an immunomodulatory and antineoplastic agent primarily used in the treatment of relapsed and refractory multiple myeloma; and **carbamazepine**, an anticonvulsant primarily used to treat epilepsy, trigeminal neuralgia, and bipolar disorder. - **Pomalidomide** enhances T-cell and NK cell-mediated immunity, inhibits production of pro-inflammatory cytokines by monocytes, induces apoptosis, and inhibits angiogenesis in vitro. - **Carbamazepine** acts mostly by inhibiting voltage-gated sodium channels in neuronal cell membranes, thereby stabilizing hyperexcited nerve membranes, inhibiting repetitive neuronal firing, and reducing synaptic propagation of excitatory impulses. There is no clinical indication or marketed product for the specific combination of these agents; however, their co-administration could occur in patients with multiple comorbidities (e.g., multiple myeloma and epilepsy). According to available pharmacokinetic data, carbamazepine (a CYP3A/P-gp inducer) does not significantly impact pomalidomide exposure in healthy individuals, suggesting a low risk of significant pharmacokinetic interaction[3]. However, clinical caution is warranted due to the serious adverse effects and the potential for pharmacodynamic interaction (e.g., additive bone marrow suppression, neurological toxicity)[2][3][5].
