Overview

The PPS regimen is a combination therapy consisting of the immunomodulatory drug pomalidomide, the anti-PD-1 monoclonal antibody tislelizumab, and the selective inhibitor of nuclear export (SINE) selinexor. This regimen is being investigated by Beijing Tongren Hospital for the treatment of relapsed or refractory primary central nervous system diffuse large B-cell lymphoma (PCNSL-DLBCL). Pomalidomide acts as a molecular glue that binds to the cereblon (CRBN) E3 ubiquitin ligase complex, leading to the degradation of transcription factors such as Ikaros and Aiolos. Tislelizumab is a humanized IgG4 monoclonal antibody that binds to the programmed cell death protein 1 (PD-1) receptor, blocking its interaction with PD-L1 and PD-L2 to restore T-cell antitumor activity. Selinexor inhibits exportin-1 (XPO1), preventing the nuclear export of tumor suppressor proteins and the glucocorticoid receptor, thereby inducing apoptosis in malignant cells. The combination aims to provide a synergistic therapeutic effect by targeting immune evasion, nuclear transport, and protein degradation pathways.
