PORT-7 + Keytruda is a combination therapy consisting of PORT-7 (formerly TT-4), a selective small molecule adenosine 2B receptor (A2BR) antagonist, and Keytruda (pembrolizumab), a programmed cell death protein 1 (PD-1) blocking monoclonal antibody. Developed by Portage Biotech in collaboration with Merck, this combination is designed to target the immunosuppressive tumor microenvironment. Adenosine, which is often elevated in the tumor microenvironment, suppresses immune cell activity via A2BR; PORT-7 aims to block this pathway to reduce immune suppression, while Keytruda enhances T-cell mediated anti-tumor responses by inhibiting the PD-1 checkpoint. The therapy is primarily being investigated for 'A2B high' solid tumors, including prostate, renal, head and neck, colorectal, endometrial, ovarian, and non-small cell lung cancers, particularly in patients who may be resistant to standard checkpoint inhibitor monotherapy.
