Potassium citrate + citric acid is a small molecule combination drug that functions as a urinary alkalinizing agent. It is composed of potassium citrate and citric acid monohydrate, typically available as an oral solution or crystals for reconstitution. The drug's mechanism of action involves the systemic metabolism of citrate to bicarbonate, which is subsequently excreted in the urine, thereby increasing urinary pH and citrate levels. This alkalinization reduces the acidity of the urine and increases the solubility of stone-forming salts, effectively inhibiting the crystallization of calcium oxalate, calcium phosphate, uric acid, and cystine. It is primarily indicated for the long-term management of renal tubular acidosis, hypocitraturic calcium oxalate nephrolithiasis, and the prevention of uric acid and cystine kidney stones, including those associated with gout.
