Overview

This is a combination therapy consisting of a proton pump inhibitor (PPI), amoxicillin, and levofloxacin. It is used primarily as a second-line or salvage regimen for the eradication of Helicobacter pylori infection, particularly in cases where first-line therapies (such as PPI + amoxicillin + clarithromycin) have failed due to antibiotic resistance. The mechanism involves suppression of gastric acid secretion by the PPI, which enhances the effectiveness of antibiotics against H. pylori; amoxicillin acts as a broad-spectrum beta-lactam antibiotic inhibiting bacterial cell wall synthesis; and levofloxacin is a fluoroquinolone antibiotic that inhibits bacterial DNA gyrase and topoisomerase IV, disrupting DNA replication and transcription. This triple therapy is typically administered for 7–14 days[1][2][4].
