**pramipexole + aprepitant** is an experimental combination of two small molecule pharmaceutical drugs: pramipexole, a dopamine D2/D3 receptor agonist primarily indicated for Parkinson's disease and restless legs syndrome, and aprepitant, a neurokinin-1 (NK1) receptor antagonist primarily approved for chemotherapy-induced and postoperative nausea and vomiting. This combination has been investigated for use in neurodegenerative disorders and in reducing pramipexole-induced adverse effects, potentially enabling safe, higher dosing of pramipexole for Parkinson’s disease and related disorders as well as for neuroprotection and symptomatic treatment[2][3]. Preclinical and early patent literature suggest that NK1 antagonists such as aprepitant may enhance pramipexole efficacy and tolerability, possibly by counteracting central and peripheral side effects and improving alteration of disease biomarkers such as alpha-synuclein, thus achieving higher doses or new treatment horizons for synucleinopathies (e.g., Parkinson’s disease)[2][3].\nThis combination currently does not have an approved brand or fixed-dose formulation but is pursued either as co-administration or in intellectual property for fixed-dose combinations.
