Overview

Prasugrel + bivalirudin is a combination of two pharmaceutical agents used as adjunct antithrombotic therapy, primarily in patients with ST-segment elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI). Prasugrel is an oral P2Y12 receptor antagonist that inhibits platelet activation and aggregation, while bivalirudin is an intravenous direct thrombin inhibitor that prevents thrombus formation by inhibiting both free and clot-bound thrombin. The combination aims to maximize antithrombotic efficacy and safety by targeting different pathways in the coagulation cascade. This regimen has been investigated for its potential to improve clinical outcomes compared to traditional combinations such as clopidogrel plus heparin[1][2][3].
