Pravastatin + colesevelam is a fixed-dose combination of two lipid-lowering agents used to treat hypercholesterolemia and reduce cardiovascular risk. Pravastatin is an HMG-CoA reductase inhibitor (statin) that lowers cholesterol by inhibiting the hepatic enzyme responsible for cholesterol biosynthesis, leading to increased LDL receptor expression and enhanced clearance of LDL from the bloodstream[4]. Colesevelam is a bile acid sequestrant that binds bile acids in the intestine, preventing their reabsorption; this depletion stimulates conversion of cholesterol into bile acids and further upregulates hepatic LDL receptors, also resulting in lower serum LDL-cholesterol[8][6]. The combination provides additive effects on lowering total and LDL cholesterol compared to either agent alone. Both drugs are approved for use as adjuncts to diet and exercise in adults with primary hyperlipidemia or mixed dyslipidemia, and colesevelam may also be used for glycemic control in type 2 diabetes[1][2][5].
