**Pravastatin + fenofibrate** is an oral fixed-dose lipid-modifying combination containing pravastatin sodium and fenofibrate, marketed as Pravafenix and Pravafen. Pravastatin competitively inhibits hepatic HMG-CoA reductase to reduce cholesterol synthesis and circulating LDL cholesterol, while fenofibrate is converted to fenofibric acid, a peroxisome proliferator-activated receptor alpha agonist that enhances triglyceride-rich lipoprotein metabolism and increases HDL cholesterol. The combination is approved for adult patients with mixed dyslipidaemia at high cardiovascular risk whose LDL cholesterol is adequately controlled on pravastatin but who require further triglyceride reduction and HDL cholesterol improvement.
