prednisolone + acetylcysteine (also known as C-NAC) is an investigational combination drug regimen evaluated for the treatment of severe acute alcoholic hepatitis. The regimen consists of oral prednisolone, a synthetic corticosteroid, and intravenous N-acetylcysteine (NAC), an antioxidant and glutathione precursor. The biological rationale for this combination is that severe alcoholic hepatitis is characterized by both a profound inflammatory response and a depletion of mitochondrial glutathione, a critical cellular antioxidant. Prednisolone acts to suppress the inflammatory cascade, while NAC helps replenish glutathione stores, potentially making hepatocytes more resistant to oxidative stress and TNF-alpha-mediated injury. In a multicenter randomized controlled trial (NCT00863785) led by researchers at Amiens University Hospital, the combination therapy demonstrated improved 1-month survival and a reduction in deaths due to hepatorenal syndrome at 6 months compared to prednisolone alone, although the primary endpoint of 6-month survival was not significantly improved.
