This combination therapy consists of the systemic glucocorticoid prednisolone and the vitamin D analog cholecalciferol (Vitamin D3). It was evaluated by the Post Graduate Institute of Medical Education and Research (PGIMER), Chandigarh, for the treatment of allergic bronchopulmonary aspergillosis (ABPA) in patients with asthma. Prednisolone serves as the standard-of-care anti-inflammatory agent to suppress the Th2-mediated hypersensitivity response to Aspergillus fumigatus spores that characterizes the disease. Cholecalciferol is added as an immunomodulatory supplement to potentially enhance the efficacy of glucocorticoids and address the high prevalence of vitamin D deficiency in this patient population. A Phase 2/3 trial conducted at PGIMER demonstrated the efficacy of this supplemental approach in acute-stage ABPA, showing that vitamin D can suppress Th2 responses and decrease levels of Th2 interleukins.
