Drug intelligence / Profile preview
prednisone + cyclophosphamide + doxorubicin + carmustine (commonly known as the PCAB regimen) is a combination chemotherapy regimen used in the treatment of multiple myeloma, particularly in the relapsed or refractory setting. The regimen consists of four active agents: prednisone (a synthetic glucocorticoid that exerts anti-inflammatory and immunosuppressive effects), cyclophosphamide (an alkylating agent that crosslinks DNA), doxorubicin (an anthracycline that intercalates DNA and inhibits topoisomerase II), and carmustine (a nitrosourea alkylating agent that crosslinks DNA). Together, these agents act synergistically to induce DNA damage, inhibit DNA replication, and promote apoptosis in malignant plasma cells. PCAB is typically administered as a single-day polychemotherapy regimen, making it a convenient outpatient option or bridging therapy to novel treatments such as CAR-T cell therapy.
