Predever (often referred to in the context of the PredEver First clinical trial) is a combination therapy consisting of the corticosteroid prednisone and the mTOR inhibitor everolimus. It is being investigated as a first-line treatment for patients with newly diagnosed moderate or severe chronic graft-versus-host disease (cGvHD). Prednisone provides broad immunosuppression by binding to the glucocorticoid receptor and altering gene transcription, while everolimus specifically inhibits the mechanistic target of rapamycin (mTOR) pathway, which is essential for the proliferation of T-cells and other immune cells involved in the pathogenesis of cGvHD. The combination is intended to provide synergistic immunosuppressive effects to improve clinical response rates compared to standard steroid therapy alone.
