Prednisone + tacrolimus + mycophenolate mofetil is a triple immunosuppressive regimen investigated for the treatment of focal segmental glomerulosclerosis (FSGS). This combination therapy utilizes three distinct mechanisms to suppress the immune response: prednisone acts as a broad-spectrum corticosteroid to reduce inflammation; tacrolimus (FK506) is a calcineurin inhibitor that prevents T-cell activation; and mycophenolate mofetil (MMF) is an inosine monophosphate dehydrogenase (IMPDH) inhibitor that blocks lymphocyte proliferation. The regimen was notably evaluated in a clinical trial (NCT00956059) by Xi'an Jiaotong University to determine if a low-dose prednisone approach combined with MMF and tacrolimus could provide comparable efficacy with improved safety compared to standard-dose prednisone monotherapy in FSGS patients.
