Overview

This is a multi-agent chemotherapy regimen composed of eleven drugs, each with distinct mechanisms of action, commonly used in the treatment of hematologic malignancies such as acute lymphoblastic leukemia (ALL) and other aggressive leukemias or lymphomas. The combination includes: - Prednisone and dexamethasone (synthetic glucocorticoids) that induce apoptosis in lymphoid cells. - Vincristine (a vinca alkaloid) that inhibits microtubule formation, arresting cell division[5]. - Pegylated-asparaginase (pegaspargase), an enzyme depleting asparagine required for leukemic cell survival[4][6][7]. - Bortezomib, a proteasome inhibitor disrupting protein degradation. - Cytarabine and methotrexate, antimetabolites interfering with DNA synthesis. - Cyclophosphamide and daunorubicin/doxorubicin (anthracyclines), which cause DNA damage through alkylation or intercalation. - 6-Mercaptopurine, a purine analog inhibiting nucleotide synthesis. This intensive regimen targets multiple pathways critical to cancer cell proliferation and survival. It is typically reserved for high-risk or relapsed/refractory cases due to its toxicity profile.
