pregabalin + acetaminophen + naproxen

PAiN is an investigator-designed **multimodal analgesic co-administration regimen** comprising pregabalin, acetaminophen, and naproxen for radiation-induced mucositis pain in patients receiving radiotherapy or chemoradiotherapy for mucosal head and neck cancer. The regimen was evaluated against opioid analgesia in the randomized phase 2 OPTIMAL-HN study, with opioids permitted as rescue treatment when needed. Pregabalin targets the alpha2delta auxiliary subunit of voltage-gated calcium channels and reduces calcium-dependent neurotransmitter release; naproxen inhibits cyclooxygenase-mediated prostaglandin synthesis; and acetaminophen provides analgesia through incompletely defined, predominantly central mechanisms.
