Overview

Pregabalin + desvenlafaxine is a combination therapy being evaluated for the treatment of fibromyalgia. Pregabalin is a gabapentinoid that acts as a ligand for the α2δ subunit of voltage-gated calcium channels, thereby reducing the presynaptic release of excitatory neurotransmitters. Desvenlafaxine is a selective serotonin and norepinephrine reuptake inhibitor (SNRI) that increases the synaptic concentrations of serotonin and norepinephrine, enhancing descending inhibitory pain pathways. This combination is currently being studied by Beijing Tiantan Hospital in a multicenter clinical trial (NCT07171320) to determine if the dual mechanism of action provides superior efficacy in managing widespread pain and associated symptoms of fibromyalgia compared to pregabalin monotherapy.
