Pregabalin + methylcobalamin is a combination therapy primarily used for the management of neuropathic pain, including postherpetic neuralgia (PHN) and diabetic peripheral neuropathy. Pregabalin is a gabapentinoid that acts as a ligand for the alpha-2-delta subunit of voltage-gated calcium channels in the central nervous system, thereby reducing the release of excitatory neurotransmitters such as glutamate and substance P. Methylcobalamin, a neurologically active form of vitamin B12, serves as a cofactor for methionine synthase and is involved in the synthesis of myelin and nerve regeneration. While widely used in certain regions, clinical evidence regarding the superior efficacy of the combination over pregabalin monotherapy for PHN remains a subject of investigation, with some studies suggesting limited additional benefit. It has been studied in multicenter trials evaluating its efficacy in combination with other interventions like electroacupuncture.
