Overview

The combination of proanthocyanidin (PC) and trimetazidine has been studied for its therapeutic effects, particularly in treating radiation-induced heart damage (RIHD) in non-small-cell lung cancer (NSCLC) patients. This combination therapy has shown superior efficacy compared to trimetazidine alone in mitigating radiation-induced inflammatory responses and oxidative stress. ## Mechanism of Action Proanthocyanidin is a class of polyphenolic compounds with potent antioxidant and anti-inflammatory properties. It works by neutralizing harmful free radicals, reducing oxidative stress, and modulating pro-inflammatory cytokines. Additionally, it improves endothelial function, which is crucial for blood vessel health[6][8]. Trimetazidine, on the other hand, is an anti-ischemic metabolic agent that improves the heart muscle's ability to use glucose as fuel by inhibiting fatty acid metabolism. It alters energy substrate metabolism by inhibiting terminal enzymes in the β-oxidation pathway, enhancing glucose metabolism, and reducing myocardial damage and oxidative stress[1][4]. When combined, these two agents work synergistically to protect the heart from radiation-induced damage by: 1. Inhibiting radiation-induced inflammatory responses 2. Reducing oxidative stress 3. Improving myocardial metabolism 4. Protecting myocardial cells from apoptosis ## Clinical Evidence A prospective randomized controlled study involving 86 NSCLC patients with radiation treatment demonstrated that the combination of PC and trimetazidine was superior to trimetazidine alone in treating RIHD. The study group that received both PC and trimetazidine showed: - Lower incidence of RIHD-related clinical manifestations - Fewer RIHD-related ECG changes - Fewer RIHD-related cardiac ultrasound changes - Higher serum levels of superoxide dismutase (SOD) - Lower levels of malondialdehyde (MDA) - Lower serum levels of brain natriuretic peptide (BNP), cardiac troponin (cTnT), creatine kinase (CK), and creatine kinase isoenzymes (CKMB)[1] ## Safety Profile Trimetazidine may cause gastrointestinal discomfort, nausea, vomiting, and rarely thrombocytopenia, agranulocytosis, and liver dysfunction. However, these adverse reactions tend to disappear after withdrawal[7]. Proanthocyanidin may cause gastrointestinal discomfort, headache, and skin rash. It may interact with anticoagulants, antiplatelet medicines, certain chemotherapy agents, and iron supplements[6]. The combination therapy appears to be well-tolerated, with the study concluding that "the combination of PC and trimetazidine is a reliable approach to treat NSCLC complicated with RIHD"[1]. ## Conclusion The combination of proanthocyanidin and trimetazidine represents a promising therapeutic approach for treating radiation-induced heart damage in non-small-cell lung cancer patients. By targeting both inflammatory and oxidative stress pathways, this combination therapy offers superior protection compared to trimetazidine alone.
