Prolact+ H2MF is a human milk-based human milk fortifier (HMBF) developed by Prolacta Bioscience, specifically designed for the nutritional management of extremely preterm infants (typically those born weighing less than 1,250 grams). It is the first commercially available fortifier derived entirely from pasteurized donor human milk, providing concentrated protein, calories, and minerals to support growth while allowing the infant to remain on an exclusive human milk diet (EHMD). The product is used to avoid the potential complications associated with bovine milk-based fortifiers (BMBF), such as necrotizing enterocolitis (NEC) and late-onset sepsis. While early observational data suggested significant clinical benefits, recent large-scale randomized controlled trials, including the N-HMBF study in Sweden, have shown that H2MF may not significantly reduce the composite incidence of NEC, sepsis, or mortality when compared to bovine-based fortifiers in infants already receiving an exclusive human milk diet.
