Promitil + bevacizumab is an experimental combination therapy under investigation primarily for metastatic colorectal cancer. **Promitil** is a pegylated liposomal formulation of a mitomycin-C lipidic prodrug (MLP), designed to enhance tumor delivery and reduce systemic toxicity relative to standard mitomycin-C by using a liposomal carrier. Combined with **bevacizumab**, a monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), this regimen aims to exploit both cytotoxic and anti-angiogenic effects in tumors. Clinical studies have evaluated the combination for safety and disease control, notably in patients with heavily pretreated metastatic colorectal cancer, showing a manageable safety profile and signs of antitumor activity. Promitil is developed by Lipomedix, while bevacizumab is developed by Genentech/Roche. This combination is not a marketed product, but is used in clinical trials. Mechanism of action: Promitil delivers and releases mitomycin-C selectively in tumors for direct DNA crosslinking and cytotoxicity. Bevacizumab binds to and neutralizes VEGF-A, inhibiting tumor angiogenesis and the vascular supply to cancer cells[1][3][7][9].
