This regimen includes Class IC agents (propafenone, flecainide) and Class III agents (sotalol, amiodarone), which are selected based on the presence or absence of structural heart disease in patients with paroxysmal atrial fibrillation (PAF). These small molecule drugs function by inhibiting various cardiac ion channels—specifically sodium and potassium channels—and antagonizing adrenergic receptors to stabilize the cardiac membrane and maintain sinus rhythm. The regimen was specifically evaluated in the context of managing patients who experienced recurrence of AF after an initial pulmonary vein isolation (PVI) procedure.
