This regimen involves the administration of high doses of propofol, fentanyl, and sevoflurane, specifically titrated to maintain a deep level of hypnosis as indicated by a Bispectral Index (BIS) value of 45 or lower. The study was designed to compare the physiological and immunological impacts of deep versus shallow anesthesia in patients undergoing shoulder surgery, focusing on changes in cerebrospinal fluid protein expression and the activity of immune cells such as T-cells and NK-cells. The protocol combines the GABA-A receptor modulating effects of propofol and sevoflurane with the analgesic properties of the mu-opioid receptor agonist fentanyl to achieve a profound state of general anesthesia.
