The combination of propofol, remifentanil, and dexmedetomidine is a multi-drug regimen used for total intravenous anesthesia (TIVA). Propofol is a short-acting intravenous anesthetic that functions as a positive allosteric modulator of the GABA-A receptor, inducing sedation and hypnosis. Remifentanil is a potent, ultra-short-acting synthetic opioid that acts as a mu-opioid receptor agonist, providing intense intraoperative analgesia with a rapid onset and offset. Dexmedetomidine is a selective alpha-2 adrenergic agonist that provides sedative and analgesic-sparing effects without significant respiratory depression. This combination is often studied to optimize anesthesia depth, as measured by the Bispectral Index (BIS), and to improve the quality of recovery and postoperative pain management in surgical procedures such as unilateral knee replacement.
