Overview

This is a four-drug combination therapy consisting of a proton pump inhibitor (PPI), clarithromycin, amoxicillin, and metronidazole. It is primarily used for the eradication of Helicobacter pylori infection, especially in cases where resistance to either clarithromycin or metronidazole may be present. The PPI component (such as omeprazole, lansoprazole, pantoprazole, rabeprazole, or esomeprazole) reduces gastric acid secretion by inhibiting the H+/K+ ATPase enzyme in gastric parietal cells. Clarithromycin and amoxicillin are antibiotics that inhibit bacterial protein synthesis and cell wall synthesis respectively; metronidazole disrupts DNA synthesis in anaerobic bacteria. This quadruple regimen—often called "concomitant therapy"—is designed to maximize eradication rates by targeting H. pylori with multiple mechanisms and overcoming antibiotic resistance[3][1].
