PPI + octreotide is an investigational combination therapy being evaluated for the management of acute non-variceal gastrointestinal bleeding (NVGIB), particularly in intensive care settings. The combination consists of a proton pump inhibitor (PPI), which suppresses gastric acid secretion to stabilize blood clots, and octreotide, a synthetic somatostatin analogue. Octreotide acts by inhibiting the secretion of gastrointestinal hormones like gastrin and secretin, reducing splanchnic blood flow, and decreasing intestinal motility. This dual approach is hypothesized to be more effective than PPI monotherapy in preventing rebleeding by both reducing the acid-mediated degradation of clots and decreasing the vascular pressure in the gastrointestinal tract. It is currently being studied in a Phase 2/3 trial (NCT06062719) sponsored by the National Hepatology & Tropical Medicine Research Institute.
