PRP + PL is an autologous orthobiologic combination therapy developed by Regenexx, LLC, for the treatment of axial lower back pain associated with multifidus muscle atrophy. The treatment protocol involves a multi-modal injection approach: autologous platelet-rich plasma (PRP) is injected into the lumbar multifidus muscle and facet joints, while autologous platelet lysate (PL) is administered into the epidural space. PRP provides a concentrated source of platelets that release a variety of growth factors (such as PDGF, TGF-β, and VEGF) over time to promote tissue repair and angiogenesis. Platelet lysate, created by lysing platelets to immediately release their internal growth factors and enzymes, provides a more rapid anti-inflammatory and regenerative effect. This combination therapy is designed to reverse muscle atrophy and stabilize the spine by stimulating the body's natural healing mechanisms in the affected spinal structures.
