PY159 + pembrolizumab is a combination therapy under investigation for the treatment of advanced solid tumors, including platinum-resistant ovarian cancer. PY159 is an afucosylated humanized monoclonal antibody that specifically targets Triggering Receptor Expressed on Myeloid Cells 1 (TREM1). By binding to TREM1, PY159 reprograms tumor-associated myeloid cells from an immunosuppressive to a pro-inflammatory phenotype, thereby promoting anti-tumor immune responses. Pembrolizumab is a well-established anti-PD-1 monoclonal antibody (marketed as Keytruda) that blocks the programmed death receptor-1 (PD-1) pathway, enhancing T-cell mediated immune responses against cancer cells. The combination aims to synergistically modulate both myeloid and lymphoid compartments of the tumor microenvironment to improve anti-tumor efficacy. Clinical studies have shown that this combination has an acceptable safety profile and preliminary evidence of disease stabilization in heavily pretreated patients with solid tumors[3][4][5][6][8].
