Pyrantel + oxantel is a combination anthelmintic therapy composed of two tetrahydropyrimidine derivatives. Pyrantel is a broad-spectrum agent effective against common nematodes such as *Ascaris lumbricoides* (roundworm) and hookworms, while oxantel, an m-oxyphenol analog, provides targeted high efficacy against *Trichuris trichiura* (whipworm). Both components act as agonists at nicotinic acetylcholine receptors (nAChR) on the body-wall muscle of parasites, inducing persistent depolarization and spastic paralysis, which results in the detachment and expulsion of the worms from the host's gastrointestinal tract. The combination is characterized by poor intestinal absorption, ensuring high local concentrations at the site of infection and a favorable safety profile. University Ghent has conducted Phase 4 clinical research in Ethiopia evaluating this combination, often in conjunction with mebendazole, to improve treatment outcomes for trichuriasis in schoolchildren where standard single-dose benzimidazole treatments show limited effectiveness.
