Quemliclustat + zimberelimab is an investigational combination therapy being studied for the treatment of advanced or metastatic pancreatic cancer. Quemliclustat is a potent and selective small molecule inhibitor of CD73, an ectoenzyme responsible for producing immunosuppressive adenosine in the tumor microenvironment, which can hinder the immune response to cancer. By blocking CD73, quemliclustat aims to reduce adenosine-mediated immunosuppression and enhance anti-tumor immune activity. Zimberelimab is a monoclonal antibody that targets programmed cell death protein 1 (PD-1), acting as an immune checkpoint inhibitor and thereby augmenting T cell-mediated responses against tumors. The combination is under clinical investigation, including in phase I/II and III trials, typically alongside chemotherapy regimens such as gemcitabine and nab-paclitaxel or mFOLFIRINOX. The principal developer for both agents in combination is Arcus Biosciences in collaboration with Gilead Sciences.
