Overview

Quetiapine + loxapine is a combination of two antipsychotic drugs used primarily in the management of schizophrenia and other psychotic disorders. Quetiapine is an atypical antipsychotic that acts mainly as an antagonist at serotonin 5-HT2A receptor and dopamine D2 receptor, with additional antagonism at histamine H1 receptor and adrenergic α1 receptor. Loxapine, while classified as a typical (first-generation) antipsychotic, also exhibits atypical properties due to its high-affinity antagonism of both dopamine D2 receptor and serotonin 5-HT2A receptor. Loxapine also blocks histaminergic H1 receptor, cholinergic M1 receptor, and adrenergic α1 receptor[3][4][5][6]. Both drugs are orally administered small molecules developed for the treatment of schizophrenia; quetiapine is also indicated for bipolar disorder and adjunctive therapy in depression[3]. The combination may be considered when switching between agents or managing complex cases where monotherapy has been insufficient.
