Quetiapine + risperidone is a combination of two atypical (second-generation) antipsychotic drugs. Both agents are primarily used to treat psychiatric disorders such as schizophrenia and bipolar disorder. Quetiapine acts mainly by antagonizing serotonin 5-HT2A receptor and dopamine D2 receptor, while risperidone also blocks these receptors with high affinity, particularly 5-HT2A, and has moderate activity at adrenergic alpha1 receptor. The combination is not standard clinical practice due to overlapping mechanisms and increased risk of adverse effects, including additive central nervous system depression and potential for serious cardiac arrhythmias due to QT interval prolongation[1][2][4]. Each drug is metabolized differently: quetiapine via CYP3A4 and risperidone via CYP2D6.
