R1507 + erlotinib + gemcitabine is a combination of three agents used as an experimental anticancer regimen. - **R1507** is a fully human IgG1 monoclonal antibody targeting the insulin-like growth factor-1 receptor (IGF-1R), blocking IGF-1/IGF-2 mediated growth signaling in malignant cells[2][3]. - **Erlotinib** is a small molecule inhibitor of the epidermal growth factor receptor (EGFR) tyrosine kinase, thus blocking downstream signaling involved in proliferation and survival[1][3][7]. - **Gemcitabine** is a nucleoside analog (antimetabolite) that inhibits DNA synthesis in replicating cells, inducing apoptosis[1][6][7]. This combination is designed to block two critical growth/survival pathways (IGF-1R and EGFR) in cancer cells, and to use gemcitabine as a cytotoxic backbone. The regimen has been evaluated in phase I/II studies, primarily in solid tumors such as non-small cell lung cancer (NSCLC) and pancreatic adenocarcinoma[2][3][5]. Preliminary studies show tolerability but no clear added efficacy over standard regimens[3]. R1507 was developed by Roche[5], erlotinib by OSI Pharmaceuticals and Genentech/Roche[7], and gemcitabine by Eli Lilly.
