**R2R01 + terlipressin** is a combination treatment under investigation for patients with hepatorenal syndrome–acute kidney injury (HRS-AKI). R2R01 is a synthetic peptide and potent, long-acting agonist of relaxin family peptide receptor 1 (RXFP1), mimicking the effects of the natural hormone relaxin to enhance cardiovascular and renal function. Terlipressin is a synthetic vasopressin analog that acts primarily as a vasoconstrictor on vasopressin receptor 1A (V1A) to improve renal perfusion. This combination is designed to synergistically address both the hemodynamic and endothelial components of HRS-AKI: R2R01 to improve renal blood flow through vasodilation and endothelial function, and terlipressin to counteract splanchnic vasodilation and increase effective arterial blood volume[1][2][3][4][5][6].
