This bismuth-based quadruple therapy is a combination regimen investigated by Shanghai East Hospital for the rescue treatment of Helicobacter pylori infection. The regimen consists of rabeprazole (a proton pump inhibitor), colloidal bismuth pectin, cefuroxime (a second-generation cephalosporin antibiotic), and metronidazole. It is designed as a 14-day oral treatment for patients who have failed previous eradication attempts. Rabeprazole serves to suppress gastric acid production, thereby increasing the stability and efficacy of the antibiotics. Colloidal bismuth pectin provides direct antimicrobial activity and mucosal protection. Cefuroxime and metronidazole act as bactericidal agents, with cefuroxime inhibiting bacterial cell wall synthesis and metronidazole causing DNA damage in the microaerophilic H. pylori bacteria.
