Overview
Rabeprazole + nortriptyline is an investigational combination regimen studied for the treatment of refractory gastroesophageal reflux disease (GERD). The regimen consists of rabeprazole, a proton pump inhibitor (PPI) that suppresses gastric acid production by irreversibly inhibiting the H+/K+ ATPase enzyme (the gastric proton pump), and nortriptyline, a tricyclic antidepressant (TCA) that acts as a visceral analgesic and neuromodulator. This combination targets both the physiological acid-related symptoms and the esophageal hypersensitivity or functional pain components that often contribute to persistent symptoms in patients who fail standard PPI therapy. The efficacy of this combination has been evaluated in randomized clinical trials sponsored by the US Department of Veterans Affairs, comparing it against double-dose PPI therapy and standard-dose PPI plus placebo.
