Oxaliplatin is a platinum-based chemotherapy drug administered intravenously that destroys quickly dividing cells, such as cancer cells[2][8]. ## Clinical Development This combination has been investigated in clinical trials, particularly for advanced gastric cancer. A Phase I trial was conducted at Asan Medical Center to evaluate the safety and efficacy of this combination[4]. The study aimed to determine the maximum tolerated doses and toxicities of the combination[1][4]. The dosing regimen in clinical trials typically involved: - RAD001 (everolimus) administered daily - Capecitabine administered on days 1-14 of a 21-day cycle - Oxaliplatin administered intravenously[5] Different dose levels were tested to find the optimal balance between efficacy and toxicity[5]. For example, one study examined dose levels ranging from 2.5mg to 10mg twice daily for everolimus, combined with varying doses of capecitabine[5]. ## Mechanism of Action The combination leverages multiple mechanisms to target cancer cells: 1. RAD001 (everolimus) inhibits the mTOR pathway, which is aberrantly activated in many cancers, including gastric cancer[1][5]. 2. Capecitabine is converted to 5-fluorouracil in the body, which interferes with DNA synthesis and cell division. 3. Oxaliplatin forms platinum-DNA adducts that inhibit DNA replication and transcription, leading to cell death[2]. This multi-targeted approach may provide enhanced efficacy compared to single-agent treatments, particularly in patients who have developed resistance to other therapies. The combination of capecitabine and oxaliplatin alone (known as CAPOX or XELOX) is already an established treatment for several cancers, including bowel, esophageal, and stomach cancers[2][8]. The addition of RAD001 aims to improve outcomes by targeting the mTOR pathway, which is involved in cancer cell growth, proliferation, and survival[1][9].
