Overview

Radiotherapy + DICEP is a sequential treatment protocol developed by Fudan University Shanghai Cancer Center for the management of newly diagnosed, early-stage (IE/IIE) extranodal NK/T-cell lymphoma (ENKTL), nasal type. The regimen begins with involved-field radiotherapy (IFRT) at a total dose of 50 Gy, followed by three cycles of the DICEP combination chemotherapy. DICEP consists of dexamethasone, ifosfamide, cisplatin, etoposide, and pegaspargase. This protocol is specifically designed to overcome the multi-drug resistance (MDR) often seen in NK/T-cell lymphomas, which frequently overexpress the P-glycoprotein. The inclusion of pegaspargase is vital as it depletes L-asparagine, an amino acid essential for the survival of NK/T-cell lymphoma cells, bypassing MDR-mediated resistance. Clinical trial results have demonstrated high efficacy, with reported 5-year overall survival and progression-free survival rates exceeding 85%.
