Radium-223 dichloride + abiraterone acetate is a pharmaceutical combination used in the treatment of metastatic castration-resistant prostate cancer (mCRPC) with bone metastases. Radium-223 dichloride is a targeted alpha-emitting radiopharmaceutical that selectively binds to areas of increased bone turnover associated with bone metastases, emitting alpha particles that induce double-stranded DNA breaks, leading to cancer cell death. Abiraterone acetate is a small molecule inhibitor of CYP17, an enzyme critical in androgen biosynthesis, thereby reducing androgen production and slowing tumor growth. The combination was investigated for additive or synergistic efficacy; however, clinical trials demonstrated increased risk of bone fractures and did not show a survival benefit over abiraterone alone, leading to recommendations against their routine combined use[1][3][5]. The combination is not an approved standard of care, though ongoing studies continue to explore optimal sequencing and settings[3][5].
